This form assigns primary responsibility for Hamilton Township School District IT equipment to the borrower.  The borrower will be responsible for taking the necessary precautions to protect the equipment and to store it in a manner that provides adequate protection when it is not in use, thus not subjecting the equipment to possible theft or damage.  If it is determined that loss or damage is a result of negligence, the borrower may be held financially responsible for the repair or replacement of the equipment.

This equipment is the property of Hamilton Township School District and therefore must be used for school business only. You are the only person authorized to use this equipment while you have signed it out. Under no circumstances should you let other persons who have not signed this agreement use or otherwise handle the equipment.

We ask that you NOT install software of any kind, other than software provided by the IT department. The Hamilton Township School District Technical Support Staff will be available to provide assistance when needed.

You must have a brief orientation with one of the IT Staff before you are allowed to sign out the equipment.

**Equipment borrowed over vacations and holidays must be returned first thing upon returning to school. **Equipment borrowed over a summer vacation should be returned at least 3 weeks prior to the return of staff to the school.

If you have any problems with this equipment, lose it, or have it stolen, please contact the IT Staff (609.476.6290). DO NOT attempt to fix or alter any hardware as this may void any warranties or service contracts.
